FAQ

  • Please submit an email through our connect page by clicking here, or click the link at the bottom of the page. We will then set up a phone call (approximately 10-15 minutes) to learn more about why you are seeking services and to determine if our practice can assist you. We may then schedule an initial consultation session to learn more about you, identify goals for treatment, and discuss our recommendations. Or, if you are seeking a neuropsychological evaluation, we will start with our initial intake.

  • Contact us. Our fees vary based on the service and the experience of the provider.

  • We kindly request a minimum of 24 hours notice to cancel your appointment. If you do not cancel your appointment at least 24 hours ahead of time or do not show for your appointment, you will be charged for the cost of service. This is to ensure that we are able to offer services to others should you be unable to attend. Please note that insurance companies do not typically reimburse for no show or late cancellation appointments.

  • CPT (Procedure) Codes: If you confirm services at Connected Minds NYC, we will send you a Good Faith Estimate that includes the service codes for your specific treatment. This is not an exhaustive list. Included are the most frequently used service codes. All telehealth sessions include a 95 modifier.

  • No. We do not accept insurance. We also do not participate in single case agreements with insurance companies. Please note that if you are working with a trainee therapist, they are not licensed, and insurance will not provide out of network reimbursement for their services. It is best to call your insurance company before making an appointment so that you can fully understand what the cost will be to you. A sample conversation you can have with your insurance company could be:

“I am going to a clinical psychologist for an initial intake and likely follow up with weekly psychotherapy. She does not accept any insurance so I would like to know what my out of network benefits are. The CPT codes that she will most likely use will be 90791, 90834, and 90847. Based on that, can you tell me what percentage/dollar amount I can expect to be reimbursed?"

    Some follow up questions:

    1. Is there a cap (maximum amount) you will pay for an office visit/given year?

    2. Do I have a deductible I must first meet? If so, how much is that?

    3. Do I need preauthorization? If so, can you grant me preauthorization to see this provider with the information I just gave you.

    4. Is there a specific claim form I must use to submit claims and if so, where can I access that?

    5. How much time after a doctor’s visit do I have to submit a claim?”

  • No. We do not accept Medicare. If you are currently enrolled in Medicare and wish to be seen by us, we can enter into a private contract in which you will be charged regular fees. We can provide you with a copy of the Medicare opt-out affidavit upon request.

  • We try hard to meet the needs of all patients. While none of the doctors accept insurance or Medicare, we will provide any necessary paperwork to submit to your insurance company if you pursue out of network benefits. While insurance companies may provide reimbursement, as is the case with any out-of-network psychologist, there are no guarantees for reimbursement. If your insurance denies reimbursement and working with us becomes cost-prohibitive, please inquire about our sliding scale policy. Superbills are provided to all of our patients on a monthly basis.

If you are seeing a therapist trainee, insurance will not reimburse for the services they provide. Instead of a Superbill, we will provide you with a receipt for payment you made.

  • You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees. Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call 1-800-985-3059.
